Warranty Policy
1. Subject to the terms and conditions hereof, Control-X Medical, Inc. warrants to the original purchaser that the equipment is free of defects in materials and workmanship, as of the time and place of delivery by Control-X Medical, Inc.
2. Warranty does not apply to any equipment, which has been altered, changed, abused, subjected to misuse, or improperly installed. Control-X Medical, Inc. will honor parts warranty claims on warranted equipment for five (5) years from the date of invoice of the equipment to the original purchaser.
Model number and Serial numbers are required on all warranty items. If a malfunctioning part is from a total system, the generator serial number is also required. Warranty applies to the warranted equipment manufactured by Control-X Medical, Inc. for (5) five years from the date of invoice, based on a pro-rational coverage scale: year 1 and 2 the warranty covers 100%, year 3: 60% coverage, year 4: 40% coverage and year 5: 20% coverage. If the equipment has been relocated from the original installation site during the warranty period, the warranty remains valid only if the relocation of the equipment was performed by factory trained and approved technicians otherwise the warranty is voided. 3. Control-X Medical, Inc. will repair or replace, at its discretion, those parts proving defective, at no charge to the Dealer, F.O.B. Columbus, Ohio. The warranty does not cover time or labor incurred for the repair or replacement. All defective parts shall be sent back to Control-X Medical, Inc., 1755 Atlas Street, Columbus, OH 43228 at the Dealer’s expense. Failure to return defective parts within 30 days may invalidate the warranty and those non-returned parts will be charged to the dealer at regular dealer prices.
4. Control-X Medical, Inc. administers the vendor warranties with respect to equipment and components manufactured by others as the warranty for the respective products. (See Appendix II. for products covered)
5. Control-X Medical, Inc. disclaims all other warranties expressed or implied, as to the quality of any goods, including implied warranties of merchantability and fitness for particular purposes.
6. Under no circumstance, will Control-X Medical, Inc. be liable for special or consequential damages. Whether based upon lost goodwill, lost resale, profits, work stoppage, impairment of other goods, or otherwise and whether arising out of the breach of warranty, breach of contract, negligence or otherwise, except only in the case of personal injury where applicable law requires such liability.

The following components are excluded from the Control-X Medical, Inc. 5 years warranty policy and are sold subject to the warranties of their respective manufacturers:
− Personal computers and monitors
− High frequency generators, other than specified above
− Halo Overhead tube crane
− X-ray tubes (typically one-year manufacturer defects warranty only)
− High voltage cables
− Collimators
− AEC Chambers
− Buckys, Trays, Grids
− All other non-listed items vended by Control-X Medical, Inc. but manufactured by other manufacturers.
Warranty and Non-Warranty Replacement Parts
Ordering Policy
1. When contacting our engineering and service department please be prepared with the following information: a. The model and serial number of the unit in question. If in an emergency situation when you do not have the serial number of the unit, and if the required part is a generic type and it is available, we will provide it for you. We will invoice you for the full dealer price of the part(s). Once the serial number is received, a determination as to the applicable warranty value will be established and an appropriate credit will be issued.
b. Any and all error messages received and a brief history of the problem and any corrective measures taken prior to the parts request.
2. You may telephone the order into us but a written confirmation of the ordered part(s) including the above information in Section 1: a. and b. must be sent to our office. Failure to do this will result in your being invoiced for the full dealer price of the part(s).
3. All replacement parts will be shipped EXW Columbus, Ohio. All parts replaced under warranty must be returned to Control-X Medical, Inc. at the expense of the dealer. Returned part(s) must be accompanied with a packing list showing the following information: a. Part description,
b. Unit serial number
c. Description of the problem and when it occurred
d. RA # - You will be given a RA# (return authorization number) when you place your order which must be used when returning the replaced part(s).
Replaced parts must be returned to Control-X Medical, Inc. within 5 days and received by us within 15 days of the replacement part(s) order date unless exceptions were agreed to at the time of order by Control-X Medical, Inc. Failure to return the part(s) to us within the above time frame cannot be considered as a warranty replacement and an invoice will be issued and be valid for the full dealer price of the part.
